Hazard Report TemplatePurpose of Document

Hazard Reports function as a means of formally recording and communicating hazards that are identified in the workplace. They may also help organizations take corrective actions to address and mitigate the hazards identified.

How to Use

Staff should be trained in basic hazard identified and be given access to the Hazard Report Template / Form.  Workers can then fill out the form to formally document any hazards and the corrective actions they think are required.

This document is a template only and it must be customised for your business. Other aspects that may need to be considered include, but are not limited to, ensuring that:

  • Relevant legal requirements have been met,
  • Workplace specific risks are identified and managed, and
  • Workers are consulted with during the customisation / review process.

When to Use

This Hazard Report Template can be used when a worker identified a hazard that warrants formal documenting and requires corrective actions. An example may be a machine that has missing or damaged guarding.

Who Should Use

This form can be completed by your Safety Advisor, Project Manager or other Manager in your organisation.

Legal Considerations

There is no specific legal requirement to use Hazard Reports. However, it is best practice, especially for larger companies. It can help meet general legal duties, including:

  • s(19) WHS Act – Primary Duty of Care
  • s(47) WHS Act – Duty to Consult with Workers
  • s(34) WHS Regulations – Duty to Identify Hazards
